(P 367) AGE AS A PROGNOSTIC FACTOR IN RETROPERITONEAL SARCOMA: A MULTICENTER LATIN AMERICAN STUDY

Objective: This study aims to determine whether age, divided into specific groups, is related to prognosis and complications in patients with retroperitoneal sarcoma.

Methods: This retrospective descriptive study utilized an electronic medical records database from LATAMSARC, a collaborative group studying sarcomas in various Latin American countries, including Argentina, Chile, Costa Rica, Mexico, and Peru. The study included adult patients (≥18 years) with retroperitoneal sarcoma who underwent surgery at six cancer centers in Latin America. Key outcomes measured were surgical treatment, overall survival (OS), and progression-free survival (PFS). Patients were categorized into three age groups: Adolescents and Young Adults (AYAs) (18-39yo), adults (40-65yo), and elderly patients (>65yo). We evaluated demographic data, tumor characteristics, morbidity (Clavier-Dindo >3), and mortality. The Kaplan-Meir test and log-rank were used for survival analysis. A p-value < 0.005 was considered statistically significant.

Results: A total of 333 patients were included in the study, comprising 141 women (42.3%) and 159 men (47.7%). The location of the sarcomas was predominantly retroperitoneal (272 patients, 81.7%), with other locations including the pelvis (28 patients, 8.4%), Patients were categorized into age groups: Adolescents and Young Adults (AYAs) (38 patients, 11.4%), adults (183 patients, 55%), and elderly (71 patients, 21.3%).
Surgical approaches included compartmental resection (104 patients, 31.2%), involved organ resection (79 patients, 23.7%), and simple resection (98 patients, 29.4%). Complications classified as Clavien-Dindo grade > 3 occurred in 56 patients (16.8%), Clavien-Dindo grade > 3 complications were observed 3 being AYAs, 34 adults, and 16 elderly patients (p=0.29).
The median overall survival (OS) for AYAs was 60.84 months (95% CI 40.65-81.06), for adults 64.53 months (95% CI 58.03-71.03), and for the elderly 46.41 months (95% CI 39.47-53.37) with a p-value of 0.69 (Fig. 1). Progression-free survival (PFS) was 53.75 months (95% CI 42.71-64.78) for AYAs, 45.36 months (95% CI 37.81-52.91) for adults, and 39.66 months (95% CI 27.14-52.17) for the elderly, with a p-value of 0.21(Fig. 2).

Conclusion: This study found no statistically significant difference in OS, PFS, or complications based on age groups in patients with retroperitoneal sarcoma. Although the elderly had a lower OS and PFS, the differences were not statistically significant. Further research is needed to explore age's impact on prognosis and identify other potential prognostic factors in this patient population.
