Objective: Primary malignant bone tumors predominantly arise in pediatric, adolescent, and young adult (PAYA) patients. Definitive surgery for these tumors can lead to various degrees of limb dysfunction. PAYAs are usually interested in sports activities; however, healthcare providers may not provide sufficient information about sports after treatment. We conducted a multicenter questionnaire survey to understand the potential needs and barriers to sport among PAYA survivors of malignant bone tumors.

Methods: Survivors aged 12 to 39 years who underwent definitive surgery for primary malignant bone tumors at five specialized centers of musculoskeletal oncology between 2000 and 2020 were included. A questionnaire on sports activity was administered to examine associations with TESS (functional assessment) or EORTC QLQ-C30 (QoL assessment).

Results: Of the 72 participants, 65 (90%) expressed interest in playing sports after treatment. However, only 30 (46%) were actually able to play sport. Fourteen participants continued to play the same sport as before treatment, while 10 enjoyed a different sport. Another six had started a sport after treatment. Sports activity was not associated with TESS, QoL score, age, or type of surgery (amputation or limb-salvage surgery). Most participants (96%) enjoyed sport, and 83% were satisfied. These results were not associated with TESS or QoL scores. On the other hand, lack of opportunity, functional problems, and pain were cited as reasons for not playing sports. In addition, 74% of participants reported not receiving information about sports from their healthcare providers.

Conclusion: Although PAYA malignant bone tumor survivors were highly motivated to enjoy sports, they could not sufficiently play them. On the other hand, those who played sports could enjoy their sports regardless of the type of surgery or the degree of postoperative limb function and QoL.
